The information below will help you prepare for your appointment and get the most out of your visit.
Before Your Visit
Complete Check-In Ahead of Time
To save time in the office, we offer secure at-home and online check-in prior to your appointment.
- Complete new patient forms electronically from home
- Review and confirm your demographic and insurance information
- Upload documents when available
Completing check-in ahead of time allows our team to focus on your care, not paperwork, when you arrive.
Confirm Your Appointment Details
Please be sure you:
- Know the date, time, and location of your appointment
- Allow extra time for travel, parking, or unexpected delays
If you have questions about directions or office locations, our staff is happy to help.
What to Bring to Your Appointment
Required Items
Please bring the following with you:
- Valid photo ID
- Current insurance card(s)
- Referral authorization, if required by your insurance plan
If a referral is required, we recommend confirming that our office has received it prior to your appointment and bringing a copy with you.
Medical Information for Your Condition
To help us provide the most accurate evaluation, please bring or arrange for the following when applicable:
- Prior medical records related to your injury or condition
- Imaging studies (X-rays, MRI, CT scans) and reports
- Any relevant lab results
We have direct access to imaging from most radiology facilities. However, it is important that we know where your imaging was performed so we can ensure that both the images and reports are available for review before your appointment.
If you were evaluated or treated elsewhere, please request records from that office at least one week prior to your visit, when possible, and confirm they were received.
To help ensure everything is ready for your visit, a Patient Care Navigator may contact you prior to your appointment to verify information or assist with records and imaging coordination.
Medications & Medical History
Please provide the following information:
- A current list of all medications, including dosage and frequency
- Over-the-counter medications, supplements, and vitamins
- A list of medication allergies
- A summary of your medical history, including prior surgeries or treatments
For your convenience, all of this information can be securely entered through our patient portal at home prior to your visit. Completing this in advance helps streamline your appointment and ensures safe, coordinated care when you arrive.
During Your Appointment
Be Ready to Talk About Your Condition
You’ll be asked questions about your symptoms, injury, activity level, and goals. Many patients find it helpful to write down:
- When symptoms began
- What makes them better or worse
- Treatments already tried
- How the issue affects daily life, work, or sports
This helps us tailor a treatment plan that fits you, not just your diagnosis.

We encourage you to come prepared with a written list of questions or concerns. You may interact with several members of our care team.

Bringing a Family Member or Friend
You’re welcome to bring a trusted adult with you to your appointment for support or to help remember details discussed.
If you need a translator, please bring an adult translator or contact our office ahead of time so we can assist. We do not recommend relying on a child to translate.
